Support work

  • Our organisation is committed to providing exceptional support services to individuals in need, catering to their diverse requirements and promoting their independence. With a range of services including, but not limited to, access to the community, personal care, household tasks, companionship and advocacy, we strive to enhance the overall quality of life for our clients

  • Assistance with daily personal activities such as bathing, dressing, grooming and toileting

  • Helping individuals with mobility aids, transfers including using hoists and other tools and providing assistance during movement

  • Support with household chores such as cooking, cleaning, laundry and grocery shopping

  • Providing companionship, emotional support and encouragement. This may involve engaging in social activities, conversation and helping individuals maintain relationships

  • Prompting administration of medication, monitoring health conditions and accompanying individuals to medical appointments 

  • Facilitating access to therapy sessions such as physiotherapy, occupational therapy and speech therapy and supporting individuals in practicing therapeutic exercises or techniques

  • Assisting with finance management, filling out forms and accessing government or community services and resources

  • Acting as an advocate on behalf of the individual with disabilities, ensuring their needs and preferences are understood and respected in various settings

  • Supporting individuals in educational settings or in finding and maintaining employment, including job coaching and assistance with workplace accommodation's

  • Providing or arranging transportation to appointments, social outings, or other activities

  • Accompanying individuals to social events, recreational activities, religious services, educational classes or cultural outings

Support coordination

  • Disability Support Coordinators are a crutial componant to ensuring that individuals receive comprehensive and effective support services. These supports also ensure that there is relief provided to informal supports such as families in advocating and supporting their loved ones. 

  • Reviewing assessments of allied health professionals to understand the needs, abilities and preferences of individuals. From these reviews, we will collate information and create an assessment to develop personalised support plans that outline the specific services and resources required to meet individual goals and to enhance their quality of life

  • We will act as a central point of contact to coordinate various support services and resources on behalf of the individual. This involves liaising with Healthcare professionals, support workers, educational institutions, employers, community organizations and government agencies as well as family and friends to ensure all aspects of the plan are implemented effectively. 

  • Advocating for the rights and interests of individuals with disabilities is at the heart of what we do. We work to ensure that individuals receive fair and equitable access to services, accommodations and opportunities in various settings such as healthcare, education, employment, housing and community activities

  • We regularly monitor the progress of your plan and review its effectiveness. We gather feedback from the participant as well as their support network and relevant stakeholders to make adjustments to the plan as needed to meet ever evolving needs 

  • We respons to crises or emergencies that may arise such as health, living or support changes. We collaborate with relevant parties to ensure timely and appropriate responses to these situations

  • We educate and train individuals, families, support workers and community members on disability awareness, rights, effective communication strategies and best practices in disability support

  • We maintain accurate records of assessments, support plans, progress notes and correspondence. We may also prepare reports for NDIS and other government bodies. 

Tutoring

  • Providing education and support for those wanting to build Auslan as a new language or just need practice and community exposure.

  • Our staff are available around Melbourne to meet you or your family for sessions.

  • Able to provide sessions virtually, through zoom or teams in the evenings for those with complex schedules.

  • Our support workers and tutors promote and advocate for community inclusion. We encourage clients to attend Deaf events with their support workers to facilitate their use of language skills.
